- Letter from shareholder reporting loss of shares to the Company.
- Shareholder can request for issue of duplicate certificate in form ISR-4 ;
- A copy of FIR including e-FIR/Police complaint/Court injunction order/copy of plaint (where the suit filed has been accepted by the Court and Suit No. has been given), necessarily having details of the securities, folio number, distinctive number range and certificate numbers (not required in case the market value of securities does not exceed Rupees Five Lakhs);
- Advertisement regarding loss of securities in a widely circulated newspaper (not required in case the market value of securities does not exceed Rupees Five Lakhs);
- Affidavit duly executed on stamp paper of Rs. 100/- in Form A and Indemnity bond duly executed on stamp paper of Rs. 500/- in Form B ;
- The applicant shall quantify the value of the securities on the basis of the closing price of such securities at any one of the recognized stock exchanges a day prior to the date of such submission in the application;
Note: The shareholder shall submit all the KYC documents along with the prescribed investors service request forms.
